GHS ATHLETICS - 2010 REQUIRED FORMS * - PHYSICAL FORM, EMERGENCY FORM/CODE OF CONDUCT.
Hand completed PHYSICAL FORM to the nurse before the start of football. She is usually in her office over the summer Mon-Fri 8am-2pm, but call 625-8011 before you go. The nurse keeps your Physical Form and stamps your EMERGENCY FORM. Player and parent/guardian sign the CODE OF CONDUCT on the back. (This is the same as the yellow card you may already have. Use either one.) Hand your stamped/signed form to the coach on the first day. NO FORMS - NO PLAY.
